Satellite clinic of Iberia Comprehensive Health Care Clinic. Full service medical and dental clinic serving people from 4 weeks of age and up. Services include family medicine, internal medicine, primary health care screening, lab, physicals, general dentistry (some oral surgery), pharmacy assistance (no in-house program), patient education, referrals, disease screening, infection control, mental health/psychiatric services and immunizations. Mental health services are offered 1 day a week on Thursday by appointment only. The Center is also an enrollment center for Medicaid, with an appointment. Sliding fee scale is offered for medical and dental treatment based on family income and size. Medicaid accepted for all patients must bring proof of income (blue sheet, check stub, W-2) and family size, document to prove accurate address and phone number (current utility bill). There may be a waiting period for treatment. Dental clinic $50 emergency visit charge for Dental, accepts Medicaid if clients are under 21. Extractions are $42-$70 and surgical procedures are $75-$340. The cost for dental assessments and X-rays at the first visit is $70. Anyone under a doctor’s care must get a doctor’s release before any dental work will be done. ANYONE is eligible for services. Assistance is provided to patients in securing Pharmaceuticals through major drug company indigent programs.
